In case you are providing a bank statement as address proof, it needs to be the latest one bearing one           quarter's transactions, needs to be self attested, needs to have your address along with the bank's           address mentioned on the statement and you need to submit an original cancelled signed cheque leaf.   It is mandatory to fill in your Mobile No. & Email ID in the CMC form in case any other details are being           updated.

In case the cheque leaf is not a personalized one then you will need to submit a copy of your Latest Bank Statement (Latest quarter ended with duly attested (signed and stamped) by the authorised official of the bank mentioning the name and designation of such authorised official.) or a copy of the Passbook displaying your Address and the Latest Three Months Transactions along with a signed cancelled cheque leaf for linking the bank account.

Click here to download "Change in Signature form" and submit the same duly filled to any of our branches along with a verified copy of your photo identity proof and your latest DP transaction statement from the branch.

Please note that it is mandatory to have your banker duly attest your new signature i.e. the bank verification stamp along with the verifier's designation and code number needs to be clearly mentioned. The reason for change in the signature has to be clearly mentioned in the Change in Signature form.
